Práticas de cuidado em saúde mental na atenção básica: uma revisão integrativa de literatura

Abstract

RESUMO: A crítica ao modelo do hospital psiquiátrico foi fortemente influenciada por fatores socioculturais, pois durante todo o século XX houve uma crescente preocupação com o respeito aos direitos civis e liberdades individuais, além do questionamento dos limites do Estado no controle social dos indivíduos. A partir desses questionamentos e embates surge a Política Nacional de Saúde Mental que propõe a garantia do cuidado ao paciente com transtorno mental em serviços substitutivos aos hospitais psiquiátricos, desconstruindo a lógica das internações de longa permanência que tendem a isolar o paciente do convívio com a família e com a sociedade. Diante do exposto, a pesquisa teve como objetivo, conhecer como a literatura tem abordado a prática dos profissionais da atenção básica frente ao cuidado em saúde mental na Estratégia de Saúde da Família. E como objetivos específicos, caracterizar os documentos quanto ao periódico, base de dado, tipo de pesquisa, ano e local; identificar as potencialidades e dificuldades frente a assistência a pessoas com transtornos mentais e verificar as estratégias usadas pelos profissionais de nível superior para garantir a assistência a pessoa com transtornos mentais. A metodologia adotada foi uma revisão integrativa de literatura, realizada nas bases de dados indexadas da Biblioteca Virtual em Saúde. O recorte temporal utilizado foi de 2016 a maio de 2020. Os descritores utilizados nas buscas foram: Assistência à Saúde Mental, Atenção Primaria à Saúde, Transtornos Mentais, e os filtros: Disponíveis, Idioma- Português, Ano de Publicação- Últimos 5 anos, Tipo de documento: Artigo. Inicialmente, com o cruzamento dos descritores obteve-se um total de 5.274 artigos científicos. Após serem atendidos os critérios de inclusão, foi totalizado um universo de 52 artigos, dos quais 17 foram selecionados para compor a amostra do estudo. Os resultados evidenciaram que o diálogo e a troca produtiva entre equipes, sendo marcante a associação do matriciamento com a possibilidade de encontros produtivos é um ponto facilitador na assistência. Denotou-se a questão dos avanços na desinstitucionalização da população internada propondo subsídios e ampliando a integralidade da saúde, que é um princípio chave no cuidado. Com isso, identifica-se que uma das principais dificuldades é a deficiência na articulação da rede em saúde mental, ressaltando posturas estigmatizantes que conduz impasses na ressocialização desses indivíduos. Desse modo, as estratégias assistenciais favorecem a atração dos usuários e buscam uma maior efetividade dentro do serviço. Nesta perspectiva, conclui-se que existe a necessidade de aprofundar o conhecimento dos profissionais, gerando segurança na assistência para que entenda o papel da rede de atenção, para que em conjunto construa-se um cuidado cada vez mais acolhedor e efetivo. ________________________________________________________________________

ABSTRACT: Criticism of the psychiatric hospital model was strongly influenced by socio-cultural factors, as throughout the twentieth century there was a growing concern with respect for civil rights and individual freedoms, in addition to questioning the limits of the state in the social control of individuals. From these questions and clashes, the National Mental Health Policy emerges, which proposes the guarantee of care for patients with mental disorders in substitutive services to psychiatric hospitals, deconstructing the logic of long-term hospitalizations that tend to isolate the patient from living with the family. and with society. In view of the above, the research aims to understand how the literature has addressed the practice of primary care professionals regarding mental health care in the Family Health Strategy. And as specific objectives, characterize the documents regarding the journal, database, type of research, year and place; identify the potentials and difficulties facing assistance to people with mental disorders and verify the strategies used by higher education professionals to guarantee assistance to people with mental disorders. The methodology adopted will be an integrative literature review, carried out in the indexed databases of the Virtual Health Library. The time frame used was from 2016 to May 2020. The descriptors used in the searches were: Assistance to Mental Health, Primary Care to Health, Mental Disorders, and filters: Available, LanguagePortuguese, Year of Publication- Last 5 years, Document type: Article. Initially, with the crossing of the descriptors, a total of 5,274 scientific articles were obtained. After the inclusion criteria were met, a total of 52 articles was added, of which 17 were selected to compose the study sample. The results showed that the dialogue and the productive exchange between teams, with the association between matrix support and the possibility of productive meetings being a striking point, is a facilitating point in assistance. The issue of advances in the deinstitutionalization of the hospitalized population was proposed, proposing subsidies and expanding the comprehensiveness of health, which is a key principle in care. Thus, it is identified that one of the main difficulties is the deficiency in the articulation of the mental health network. Highlighting stigmatizing attitudes that leads to impasses in the resocialization of these individuals. Thus, assistance strategies favor the attraction of users and seek greater effectiveness within the service. In this perspective, it is concluded that there is a need to deepen the knowledge of professionals, generating security in care so that they understand the role of the care network, so that together, an increasingly welcoming and effective care is built.
