Adequação da terapia nutricional enteral em pacientes hospitalizados: uma revisão integrativa da literatura

Abstract

RESUMO: A Nutrição enteral abrange qualquer forma de suporte nutricional que tenha o alimento como finalidade medicinal, podendo ser ofertada por via oral na forma de suplementos alimentares ou por meio de sondas. Um paciente pode ser considerado em estado crítico quando está precisando urgentemente de cuidado clínico. A terapia nutricional enteral tem como finalidade promover saúde, além de diminuir o estresse fisiológico e manter a imunidade do paciente, portanto é de extrema importância que a terapia nutricional enteral ofertada corresponda à prescrita. Desta forma, este estudo tem como objetivo investigar na literatura a adequação da Nutrição Enteral prescrita com a dieta infundida em pacientes hospitalizados e como objetivos específicos: identificar a adequação do volume prescrito com o que foi infundido para o paciente, identificar a adequação proteico-calórico prescrita com a que foi infundida no paciente e identificar os fatores limitantes para a adequação. Para a realização do projeto, foi feita uma busca nas bases de dados Scientific Electronic Library Online, US National Library of Medicine National Institutes of Health, Portal de Periódicos da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior e Medical Literature Analysis and Retrieval System Online, no período de 26 de maio a 02 de junho de 2020. Neste período, de acordo com os critérios de inclusão e exclusão foram selecionados 12 artigos para a leitura do resumo, destes, 9 foram selecionados para a leitura completa e apenas 5 foram incluídos na revisão. Os resultados mostraram que a adequação nutricional dos pacientes é algo muito relativo, visto que dos 5 artigos selecionados, apenas 3 tiveram um bom resultado em relação a adequação calórica e proteica, já em relação aos fatores limitantes foi possível identificar alguns, sendo eles, diarreia, volume residual gástrico, vômito, distensão abdominal, jejum para procedimentos, náuseas, complicações clínicas, perda do acesso enteral e procedimentos cirúrgicos. Sendo assim, conclui-se que é necessária uma maior atenção aos profissionais responsáveis pela terapia nutricional enteral, tanto na hora da introdução da nutrição enteral, quanto na hora da manutenção e acompanhamento. ________________________________________________________________________________________ ABSTRACT: Enteral nutrition covers any form of nutritional support that has food as a medicinal purpose, and can be offered orally in the form of food supplements or through probes. A patient may be considered in critical condition when he is in urgent need of clinical care. Enteral nutritional therapy aims to promote health, in addition to reducing physiological stress and maintaining the patient's immunity, therefore it is extremely important that the enteral nutritional therapy offered corresponds to the prescribed one. Thus, this study has the investigation in the literature of the adequacy of enteral nutrition prescribed with the infused diet in hospitalized patients and as specific objectives: to identify the adequacy of the volume prescribed with what was infused to the patient, to identify the protein-caloric adequacy prescribed with that which was infused in the patient and to identify the limiting factors for adequacy. To carry out the project, a search was made in the databases Scientific Electronic Library Online, US National Library of Medicine National Institutes of Health, Journal Portal of the Coordination of Improvement of Higher Education Personnel and Medical Literature Analysis and Retrieval System Online, from May 26 to June 2, 2020. In this period, according to the inclusion and exclusion criteria, 12 articles were selected to read the abstract, of which 9 were selected for full reading and only 5 were included in the review. The results showed that the nutritional adequacy of the patients is something very relative, since of the 5 selected articles, only 3 had a good result in relation to caloric and protein adequacy, while in relation to limiting factors it was possible to identify some, being diarrhea, gastric residual volume, vomiting, abdominal distension, fasting for procedures, nausea, clinical complications, loss of enteral access and surgical procedures. Thus, it is concluded that greater attention is needed from the professionals responsible for enteral nutritional therapy, both at the time of the introduction of enteral nutrition, and at the time of maintenance and follow-up.
