Associação entre disfunção temporomandibular e zumbido: revisão de literatura

Abstract

RESUMO: A disfunção temporomandibular (DTM) é um conjunto de desordens que acometem o sistema estomatognático, que incluem a articulação temporomandibular (ATM), os músculos da mastigação e estruturas associadas. Estas são manifestadas principalmente por estalos ou crepitações, dificuldade durante movimento de abrir e fechar a boca e dor, que pode irradiar para regiões da cabeça, cervicais e região auricular. Distúrbios otológicos em pacientes com disfunção temporomandibular, como o zumbido, estão, em alguns casos, correlacionados entre a anatomia e a função da articulação, os músculos inervados pelo trigêmeo e estruturas do ouvido. Assim, a pesquisa traz o seguinte problema de estudo: como a literatura tem abordado a associação entre o zumbido e a disfunção temporomandibular? O objetivo deste trabalho é conhecer através da literatura como tem sido abordada a associação entre a disfunção temporomandibular e zumbido. O estudo em questão consiste em uma revisão de literatura integrativa que é constituída por artigos científicos de estudos teóricos de bases eletrônicas. O estudo bibliográfico foi realizado nas bibliotecas eletrônicas Scientific Electronic Library Online e Biblioteca Virtual em Saúde, tendo um recorte temporal de janeiro de 2015 a maio de 2020. Foram selecionados os artigos que seguem os seguintes critérios de inclusão: artigos que abordem a temática sobre a disfunção temporomandibular, zumbido, sintomas associados e seus possíveis tratamentos, com textos em português e inglês indexados nas bases de dados selecionadas, publicados no período determinado na pesquisa. Foram excluídos materiais digitais das bases: monografias, tese e manuais. Após a identificação dos títulos nos periódicos on-line foram realizadas leitura flutuante, exploratória e analítica, para selecionar as informações dos artigos. Assim o estudo poderá possibilitar um conjunto de informações bem atuais sobre a temática, podendo favorecer novos direcionamentos e reflexões ao profissional da área. Foram selecionados 19 artigos para compor os resultados desta pesquisa. Os artigos selecionados apontam que o zumbido presente em pacientes com disfunção temporomandibular, oscila entre 20,7% – 82,9% dos casos, variando suas causas e fatores etiológicos, contemplando que existe, em alguns indivíduos, associação entre disfunção temporomandibular e zumbido, principalmente com presença de pontos gatilhos miofasciais. Nos casos em que existe uma relação causal entre ambos, geralmente quando consegue-se o controle dos sintomas de disfunção temporomandibular, há uma melhora no zumbido. A realização deste estudo se tornou limitada por conta da quantidade de estudos encontrados antes do período de 2015. Mesmo sendo uma abordagem complexa, constata-se a necessidade de novos trabalhos acerca da atuação de abordagens na área de disfunção temporomandibular associado ao zumbido, para que haja uma maior contribuição de conteúdo. _______________________________________________________________________________________ ABSTRACT: Temporomandibular disorder (TMD) is a set of disorders that affect the stomatognathic system, which include the temporomandibular joint (TMJ), masticatory muscles and associated structures. These are mainly manifested by crackling or crackling, difficulty during movement to open and close the mouth and pain, which can radiate to the head, cervical and auricular regions. Otological disorders in patients with temporomandibular disorders, such as tinnitus, are, in some cases, correlated between anatomy and function of the joint, muscles innervated by the trigeminal and structures of the ear. Thus, the research brings the following study problem: how has the literature addressed the association between tinnitus and temporomandibular disorder? The objective of this work is to know through the literature how the association between temporomandibular disorder and tinnitus has been approached. The study in question consists of an integrative literature review that consists of scientific articles from theoretical studies of electronic bases. The bibliographic study was carried out in the electronic libraries Scientific Electronic Library Online and Biblioteca Virtual em Saúde, with a time frame from January 2015 to May 2020. Articles were selected that follow the following inclusion criteria: articles that address the theme on temporomandibular disorder, tinnitus, associated symptoms and their possible treatments, with texts in Portuguese and English indexed in the selected databases, published in the period determined in the research. Digital materials were excluded from the databases: monographs, thesis and manuals. After identifying the titles in the online journals, a floating, exploratory and analytical reading was performed to select the information from the articles. Thus, the study may provide a set of very current information on the subject, which may favor new directions and reflections for the professional in the area. 19 articles were selected to compose the results of this research. The selected articles point out that the tinnitus present in patients with temporomandibular disorders varies between 20.7% - 82.9% of the cases, varying their causes and etiological factors, considering that, in some individuals, there is an association between temporomandibular disorder and tinnitus, mainly with the presence of myofascial trigger points. In cases where there is a causal relationship between both, usually when control of the symptoms of temporomandibular disorder is achieved, there is an improvement in tinnitus. The realization of this study became limited due to the number of studies found before the period of 2015. Even though it is a complex approach, there is a need for new studies on the performance of approaches in the area of temporomandibular disorder associated with tinnitus, so that there is a greater contribution of content.
