A associação de métodos de diagnóstico por imagem na avaliação do linfonodo sentinela na mama

Abstract

RESUMO: O câncer de mama acomete principalmente a população feminina. Quando ocorre, a metástase, dá-se preferencialmente pela cadeia linfática. O linfonodo sentinela é o primeiro gânglio que irá receber a drenagem linfática proveniente do tumor. A drenagem da linfa mamária ocorre principalmente para a axila, por isso há uma necessidade de estudos para essa região. Os métodos de imagem permitem a identificação dos linfonodos antes e durante o processo cirúrgico. O presente trabalho teve como objetivo realizar uma revisão de literatura sobre os métodos de imagem na avaliação do linfonodo sentinela na mama. Tratou-se de uma revisão sistemática na base de dados PubMed, via NCBI, onde foram incluídos artigos com recorte temporal de cinco anos, gratuitos, em língua pátria, espanhola e inglesa, disponíveis na integra e que abordassem o tema. Excluíram-se os editoriais, patentes, relatos de caso, dissertações e teses, artigos em duplicidade, com informações insuficientes, indisponíveis na íntegra. Foram identificados 152 artigos, onde aplicado o filtro dos últimos cinco anos restaram 145 estudos. Foi realizada a leitura dos títulos e selecionou-se 50 artigos para a leitura dos resumos. Após esta etapa, foram selecionados 17 artigos para leitura na íntegra, onde foram aplicados os critérios de inclusão e exclusão, assim, foram incluídos 10 artigos para construção deste trabalho. A biópsia do linfonodo sentinela é considerada padrão ouro para estudos da axila no câncer de mama. A linfocintilografia é considerada uma técnica capaz de identificar com precisão o sistema linfático. O radiofármaco mais utilizado na análise do linfonodo sentinela é o tecnécio 99m nanocoloide, com administração perialeolar e intratumoral. O uso do radiotraçador em conjunto com o azul de metileno facilita a localização dos linfonodos. _______________________________________________________________________________________ ABSTRACT: Breast cancer mainly affects the female population. When metastasis occurs, it occurs preferentially through the lymphatic chain. The sentinel lymph node is the first ganglion that will receive lymphatic drainage from the tumor. The drainage of the breast lymph occurs mainly to the armpit, so there is a need for studies for this region. Imaging methods allow the identification of lymph nodes before and during the surgical process. The present study aimed to conduct a literature review on imaging methods in the evaluation of sentinel lymph nodes in the breast. This was a systematic review of the PubMed database, via NCBI, which included articles with a five-year time frame, free of charge, in the mother tongue, Spanish and English, available in full and that addressed the topic. Editorials, patents, case reports, dissertations and theses, articles in duplicate, with insufficient information, which were unavailable in full, were excluded. 152 articles were identified, where the filter of the last five years was applied, 145 studies remained. The titles were read and 50 articles were selected for reading the abstracts. After this stage, 17 articles were selected for reading in full, where the inclusion and exclusion criteria were applied, thus, 10 articles were included for the construction of this work. Sentinel lymph node biopsy is considered the gold standard for axillary studies in breast cancer. Lymphoscintigraphy is considered a technique capable of accurately identifying the lymphatic system. The most used radiopharmaceutical in the analysis of the sentinel lymph node is the 99m nanocolloid technetium, with perialeolar and intratumoral administration. The use of the radiotracer in conjunction with methylene blue facilitates the location of the lymph nodes.
