A inserção do fisioterapeuta na atenção primaria à saúde: análise a nível regional através do cadastro nacional de estabelecimento de saúde 20192020

Abstract

O sistema de saúde no Brasil estruturou-se ao longo dos anos passando por diversas alterações, principalmente a partir da década de 70, culminando no Sistema Único de Saúde (SUS). A sua organização ocorre a partir da Atenção Básica (AB), através da Estratégia de Saúde da Família, desenvolvendo ações de promoção e prevenção de agravos, através de intervenções individuais e coletivas. A inserção do fisioterapeuta na AB se deu a partir da criação do Núcleo de Apoio a Saúde da Família (NASF), sendo ainda um processo em construção. Os sistemas de informação em saúde são recursos disponíveis para agrupar informações necessárias para análises da situação de saúde de um território e subsidiar a tomada de decisão pelos gestores. O Cadastro Nacional de Estabelecimentos de Saúde (CNES) é um desses sistemas onde são cadastrados todos os estabelecimentos e profissional da rede pública e privada, fornecendo dados que contribuem para que o gestor melhore as políticas em saúde. Desta forma, o objetivo geral desta pesquisa foi descrever a distribuição dos fisioterapeutas inseridos na atenção primaria à saúde dos municípios da região de saúde de Cruz das Almas. Tendo como objetivos específicos: apresentar o perfil do fisioterapeuta inserido na atenção primaria à saúde dos municípios da Região de Saúde de Cruz das Almas; e comparar a distribuição dos fisioterapeutas os anos de 2019 e 2020. Tratou-se de uma pesquisa transversal exploratória, de abordagem quantitativa de caráter descritivo, utilizando-se dados secundários em base dos sistemas de Informações em Saúde, especificamente o CNES, através das análises dos seus registros no período de 2019 a 2020. Assim, foi possível verificar na Região de Saúde de Cruz das Almas, que em oito dos nove municípios há pelo menos um fisioterapeuta atuando, além disso, há uma maior contratação de fisioterapeutas do sexo feminino com vínculo temporário, sem outro tipo de vínculo empregatício; a maior quantidade de profissionais atuando ao mesmo tempo ocorreu no município de Cachoeira (cinco profissionais em 2 unidades de saúde), enquanto o município de Maragogipe não possui fisioterapeutas cadastrados na AB, foi também observado que houve uma redução do número de profissionais contratados comparando-se os anos de 2019 e 2020. O estudo se mostrou relevante por apresentar informações relacionadas à inserção do fisioterapeuta na APS da região e a redução do seu número no último ano estudado, que pode ser reflexo da Portaria nº 2.979 de 12 de novembro de 2019, o que só poderá ser validado com estudos futuros, percebendo-se a necessidade da inclusão de novos estudos sob a temática tratada. Recomenda-se a realização de estudos que e fortaleçam a atuação do fisioterapeuta em nível primário de saúde. ___________________________________________________________________________________ The health system in Brazil has been structured over the years, passing trough several changes, mainly from the 70s, resulting in the Unified Health System (SUS). Its organization occurs from the Basic Care (AB), through the Family Health Strategy, developing actions to promote and prevent diseases, through individual and collective interventions. The insertion of the physiotherapist in AB occurred after the creation of the Family Health Support Center (NASF), however, it is still a process under construction. Health information systems are resources available to group up information needed to analyze the health situation of a territory and support decision making by managers. The National Register of Health Establishments (CNES) is one of these systems, where all establishments and professionals from the public and private network are registered, providing data that contribute for the manager to improve health policies. The objective of the study is to describe the distribution of physiotherapists inserted in primary health care of the cities in the Health Region of Cruz das Almas, having as specific objectives: to present the profile of the physiotherapist inserted in primary health care in the cities of Health Region of Cruz das Almas and compare the distribution of physiotherapists in the years 2019 and 2020. An exploratory cross-sectional research was carried out, with a quantitative approach of a descriptive character, using secondary data based on the Health Information systems, specifically the CNES, through the analysis of its records in the period from 2019 to 2020. It was possible to verify in the Health Region of Cruz das Almas that in eight of the nine cities there is at least one physiotherapist working in the city, that there is a greater hiring of female physiotherapists with temporary employment , without any other type of employment relationship than the greatest number of professionals working at the same time ran in the city of Cachoeira (five professionals in 2 health units) while the city of Maragogipe does not have physiotherapists registered in the AB and that there was a reduction in the number of professionals hired comparing the years 2019 and 2020. The study proved to be relevant for presenting information related to the insertion of the physiotherapist in the APS’s region and the reduction of its number in the last year studied, which may be a reflection of Ordinance No. 2.979 of November 12th, 2019, which can only be validated with future studies, realizing it the need for the inclusion of new studies on the topic addressed. It is recommended to carry out studies that strengthen the physiotherapist's performance at the primary health level.
