Associação entre periodontite e parto prematuro no contexto da gestação de risco: revisão de literatura

Abstract

A periodontite é uma condição infecciosa crônica onde a participação da resposta imunológica do indivíduo desempenha importante papel para sua patogênese. Essa patologia acomete os tecidos de revestimento e sustentação das unidades dentarias. As principais mudanças na gestação acontecem em virtude das alterações hormonais que aumentam gradativamente conforme as semanas vão passando e diminuem da mesma forma após o parto. O presente estudo teve como objetivo geral: analisar na literatura a relação entre periodontite e partos prematuros no contexto da gestação de risco. Visto isso, teve como objetivos específicos: caracterizar os estudos quanto ao ano e tipo de estudo; identificar estratégias utilizadas para evitar os partos prematuros em mulheres com diagnóstico de periodontite e listar as fragilidades dos estudos relacionados a associação entre periodontite e parto prematuro. Este estudo consiste em uma revisão de literatura integrativa que foi constituída por artigos científicos de estudos teóricos e empíricos de bases eletrônicas, apoiando-se em leituras exploratórias e seletivas. O estudo bibliográfico foi realizado na biblioteca Medical Literature Analysis and Retrieval System Online. Os descritores foram: periodontitis, premature birth e infant, low birth weight. Teve como critérios de inclusão: artigos em inglês que abordem a temática doença periodontal e partos prematuros e que estivessem indexados na base de dados selecionada, bem como tenham sidos publicados no período de 2015 a novembro de 2020. Foram excluídos artigos que estavam fora do período estipulado e os que de alguma forma fugiam do tema principal. Ao todo, depois dos critérios de inclusão e exclusão, foram selecionados 20 artigos. No presente trabalho, 55% dos artigos evidenciaram associação entre periodontite e parto prematuro, entretanto 45% concluíram que não há associação. A literatura aborda a relação entre periodontite e parto prematuro de forma precisa e coesa, mas sem consenso nos resultados. Há duas linhas de publicações, uma que associa periodontite e parto prematuro e outra linha que não associa. ___________________________________________________________________________________ Periodontitis is a chronic infectious condition where the participation of the individual's immune response plays an important role in its pathogenesis. This pathology affects the lining and supporting tissues of the dental units. The main changes during pregnancy occur due to hormonal changes that gradually increase as the weeks go by and decrease in the same way after delivery. The present study had as its general objective: to analyze the literature on the relationship between periodontitis and preterm births in the context of high-risk pregnancy. Therefore, it had as specific objectives: to characterize the studies as to year and type of study; to identify strategies used to prevent preterm births in women diagnosed with periodontitis and to list the weaknesses of the studies related to the association between periodontitis and preterm birth. This study consists of an integrative literature review that consisted of scientific articles of theoretical and empirical studies from electronic databases, supported by exploratory and selective reading. The bibliographic study was carried out in the Medical Literature Analysis and Retrieval System Online library. The descriptors were: periodontitis, premature birth and infant, low birth weight. Inclusion criteria were: articles in English that addressed the topic of periodontal disease and premature births and that were indexed in the selected database, as well as having been published in the period from 2015 to November 2020. Articles that were outside the stipulated period and those that somehow ran away from the main topic were excluded. In all, after the inclusion and exclusion criteria, 20 articles were selected. In the present study, 55% of the articles showed an association between periodontitis and preterm birth; however, 45% concluded that there is no association. The literature addresses the relationship between periodontitis and preterm birth in a precise and cohesive way, but without consensus on the results. There are two lines of publications, one that associates periodontitis and preterm birth, and another that does not.
