Reconstrução total com prótese customizada unilateral em anquilose congênita da articulação temporomandibular: relato de caso clínico

Abstract

As patologias articulares além de afetarem uma proporção considerável de pacientes, são frequentemente associadas ao comprometimento funcional e com problemas de ordem estética de posicionamento mandibular. As cirurgias para o tratamento da anquilose podem ser executadas de formas diversas, sendo que suas abordagens visam o alívio da dor, a redução de processos inflamatórios crônicos ou agudos, melhora da função e prevenção de lesões estruturais ou progressão de certas doenças articulares. Portanto, através desta observação, o cirurgião bucomaxilofacial poderá analisar a melhor conduta a ser tomada frente a um paciente acometido por uma patologia de anquilose articular, com otimização do tempo cirúrgico através das próteses articulares customizadas e com isso trazendo maiores benefícios ao paciente. Nesse contexto, o estudo teve como objetivo demonstrar, por meio de um relato de caso clínico, como ocorreu a reconstrução total por prótese customizada unilateral em paciente diagnosticado com anquilose congênita na articulação temporomandibular. Trata-se de um estudo do tipo relato de caso de natureza descritiva, sendo a pesquisa realizada no Hospital Jorge Valente em Salvador-BA, onde foi executado o procedimento de reconstrução da articulação temporomandibular com planejamento de uma prótese aloplástica customizada. A cirurgia proposta foi realizada em um paciente com história prévia patológica de anquilose na ATM, com exame físico atípico da mobilidade mandibular e ausência de função na articulação do lado direito. Realizada sob anestesia geral, com acesso pré auricular, associado ao submandibular de Risdon, seguindo para a exposição do bloco anquilótico, ostectomia e osteoplastia com a fixação do componente fossa depois componente articular, posteriormente feita também uma coronectomia bilateral total, por acesso intraoral, com passo seguinte para verificação da melhora e queixas funcionais que o paciente apresentava. Como resultado o paciente apresentou uma melhora significativa na abertura bucal, deglutição, fala e higiene, levando assim uma melhor relação psicossocial, além de maior aceitação social. ________________________________________________________________________________________ ABSTRACT Joint pathologies, in addition to affecting a considerable proportion of patients, are often associated with functional impairment and with problems of an aesthetic nature in the mandibular position. Surgeries for the treatment of ankylosis can be performed in different ways, and their approaches aim at relieving pain, reducing chronic or acute inflammatory processes, improving function and preventing structural damage or progression of certain joint diseases. Therefore, through this observation, the maxillofacial surgeon will be able to analyze the best course of action to be taken when faced with a patient suffering from a pathology of joint ankylosis, optimizing the surgical time through customized joint prostheses and thus bringing greater benefits to the patient. In this context, the study aimed to demonstrate, through a clinical case report, how total reconstruction using a customized unilateral prosthesis occurred in a patient diagnosed with congenital ankylosis in the temporomandibular joint. This is a descriptive case-report-type study, and the research was carried out at Hospital Jorge Valente in Salvador-BA, where the reconstruction procedure of the temporomandibular joint was performed with the planning of a customized alloplastic prosthesis. The proposed surgery was performed in a patient with a previous pathological history of TMJ ankylosis, with atypical physical examination of mandibular mobility and lack of right-sided joint function. Performed under general anesthesia, with pre-auricular access, associated with Risdon's submandibular, followed by exposure of the ankylotic block, ostectomy and osteoplasty with the fixation of the fossa component, then the articular component, later also performed a total bilateral coronaectomy, by intraoral access, with the next step to verify the patient's improvement and functional complaints. As a result, the patient showed a significant improvement in mouth opening, swallowing, speech and hygiene, thus leading to a better psychosocial relationship, in addition to greater social acceptance.
