A nanotecnologia no tratamento do câncer bucal: revisão de literatura

Abstract

A incidência do câncer bucal no Brasil é uma das mais altas do mundo, os baixos índices de sobrevida refletem o seu comportamento agressivo, exigindo, por parte dos profissionais, maior atenção a esta neoplasia tanto em relação à prevenção quanto ao diagnóstico e acompanhamento. Estima-se que 90 % desses tumores são carcinomas espinocelular. A quimioterapia, a radioterapia e a remoção cirúrgica são os tratamentos mais utilizados atualmente, porém apresentam um alto índice de efeitos colaterais ao paciente. A ciência traz uma nova esperança para o tratamento oncológico através da nanotecnologia, sendo atualmente o único método capaz de realizar a entrega de drogas em local específico, sem afetar células saudáveis e com efeitos colaterais reduzidos, se comparado ao tratamento quimioterápico convencional. Nessa perspectiva, esse estudo busca responder ao seguinte problema: como se apresenta, na literatura indexada, o uso da nanotecnologia no tratamento do câncer bucal? O objetivo geral desse trabalho é comparar, na literatura indexada, o uso da nanotecnologia no tratamento do câncer bucal. Este estudo é uma revisão integrativa de literatura, composta por artigos científicos de estudos teóricos e empíricos das bases eletrônicas Biblioteca Virtual em Saúde e Sistema Online de Busca e Análise de Literatura Médica com recorte temporal de 2011 a maio de 2021, utilizando como descritores principais: nanotecnologia e nanotechnology; e para o cruzamento com o descritor principal os seguintes descritores secundários: câncer bucal, neoplasia bucal e mouth neoplasm. Foram utilizados os seguintes critérios de inclusão: documentos que apresentavam textos completos escritos nos idiomas português e inglês, indexados nas bases de dados selecionadas, e acessados da íntegra pelo meio online, no formato de artigo, tese e dissertação. Para exclusão dos artigos foi adotado como critério: documentos que não contemplavam o tema escolhido. A amostra final foi composta por 10 artigos, dos quais 50% foram publicados entre os anos de 2017 e 2018, 90% publicados em inglês e 50% publicados no continente europeu. As vantagens apresentadas pelos artigos selecionados foram: entrega direcionada, aumento da biodisponibilidade, redução dos efeitos colaterais dos fármacos, redução da toxicidade dos fármacos às células saudáveis e aumento da eficácia terapêutica. Já as limitações encontradas foram: insuficiência de estudos sobre a temática e custo relativamente alto. A pesquisa é relevante, pois visa trazer informações sobre esse tema, que representa o futuro do tratamento do câncer bucal, visto que embora represente um significativo avanço, ainda é uma temática desconhecida por grande parte da comunidade acadêmica e cirurgiões-dentistas. ________________________________________________________________________________________ ABSTRACT The incidence of oral cancer in Brazil is one of the highest in the world, the low survival rates reflect its aggressive behavior, demanding, on the part of professionals, greater attention to this neoplasm, both in terms of prevention, diagnosis and follow-up. It is estimated that 90% of these tumors are squamous cell carcinomas. Chemotherapy, radiotherapy and surgical removal are currently the most used treatments, but they have a high rate of side effects for the patient. Science brings new hope for cancer treatment through nanotechnology, being currently the only method capable of delivering drugs in a specific location, without affecting healthy cells and with reduced side effects, when compared to conventional chemotherapy treatment. From this perspective, this study seeks to answer the following problem: how is the use of nanotechnology in the treatment of oral cancer presented in the indexed literature? The general objective of this work is to compare, in the indexed literature, the use of nanotechnology in the treatment of oral cancer. This study is an integrative literature review, consisting of scientific articles from theoretical and empirical studies of the electronic databases Virtual Health Library and Online System for Search and Analysis of Medical Literature with a time frame from 2011 to May 2021, using as main descriptors: nanotecnologia and nanotechnology; and for the crossing with the main descriptor, the following secondary descriptors: oral cancer, oral neoplasm and mouth neoplasm. The following inclusion criteria were used: documents that had full texts written in Portuguese and English, indexed in selected databases, published from 2011 to 2021 and accessed in their entirety online, in the form of an article, thesis and dissertation. For exclusion of articles, the following criterion was adopted: documents that did not include the chosen topic. The final sample consisted of 10 articles, of which 50% were published between 2017 and 2018, 90% published in English and 50% published on the European continent. The advantages presented by the selected articles were: targeted delivery, increased bioavailability, reduced drug side effects, reduced drug toxicity to healthy cells, and increased therapeutic efficacy. The limitations found were: insufficient studies on the subject and relatively high cost. The research is relevant because it aims to bring information on this topic, which represents the future of oral cancer treatment, since although it represents a significant advance, it is still an unknown topic for a large part of the academic community and dentists.
