Adulto jovem em hemodiálise: repercussões do tratamento na vida do paciente com insuficiência renal crônica

Abstract

A Doença Renal Crônica é definida pela lesão nos rins persistente a um período maior ou igual que três meses, com diversas causas e variáveis fatores de prognóstico. A modalidade de terapia mais utilizada é a hemodiálise, que consiste em filtrar o sangue através de uma máquina, atuando diretamente na qualidade de vida do paciente, porém limitando a vida do mesmo. O impacto do tratamento pode ser ainda maior para o adulto jovem, por está em constante busca da sua autonomia, valorização pessoal, construção familiar e inserção na sociedade. Diante disso o objetivo geral dessa pesquisa foi analisar as repercussões do tratamento hemodialítico na vida do adulto jovem com insuficiência renal crônica e como objetivos específicos traçar o perfil sociodemográfico dos adultos jovens com insuficiência renal crônica em tratamento de hemodiálise, conhecer as mudanças ocorridas na vida do adulto jovem com insuficiência renal crônica após o início do tratamento hemodialítico e identificar as estratégias utilizadas pelo adulto jovem para melhor aceitação e adaptação ao tratamento hemodialítico. Trata-se de uma pesquisa descritiva com abordagem qualitativa, realizada com oito adultos jovens com insuficiência renal crônica de ambos os sexos que realizam a terapia hemodialítica. A técnica de coleta escolhida foi a da bola de neve, onde cada participante indicou outro por meio do seu contato telefônico, sendo o primeiro integrante da pesquisa de conhecimento dos pesquisadores. A pesquisa foi realizada através da plataforma Google Forms por meio de um questionário online contendo 20 questões mistas de fácil resolução que foram posteriormente analisados através da técnica de análise de Minayo. Neste estudo houve uma maior prevalência de participantes do sexo feminino com faixa etária variando entre 19 a 29 anos. Observou-se que o tratamento hemodialítico repercute significantemente no dia a dia do adulto jovem e que as maiores dificuldades estão relacionadas com os hábitos diários, como alimentação, diminuição da ingesta hídrica. Além disso, o tempo gasto com as sessões de hemodiálise e o mal-estar durante e após o tratamento repercutem nas atividades de lazer e no vínculo social desses indivíduos, e diante a essas dificuldades vários sentimentos negativos podem surgir como tristeza, medo e angústia, mesmo sabendo da importância da hemodiálise para a sua sobrevivência. Entretanto percebeu-se que a família, a crença religiosa e a esperança em dias melhores configuram-se como ferramentas imprescindíveis no processo de aceitação e adaptação ao tratamento hemodialítico. Evidencia-se a necessidade da participação de uma equipe de enfermagem capacitada de forma a acolher o paciente, identificando as suas dificuldades e promovendo ações de educação em saúde que visem minimizar esses sentimentos. ________________________________________________________________________________________ ABSTRACT: Chronic Kidney Disease is defined by kidney damage persisting for a period greater than or equal to three months, with several causes and variable prognostic factors. The most used modality of therapy is hemodialysis, which consists in filtering the blood through a machine, acting directly on the patient's quality of life, but limiting the patient's life. The impact of the treatment can be even greater for young adults, who are in constant search for autonomy, personal appreciation, family construction, and insertion in society. Therefore, the general objective of this research was to analyze the repercussions of hemodialysis treatment in the life of the young adult with chronic renal failure and, as specific objectives, to trace the sociodemographic profile of young adults with chronic renal failure undergoing hemodialysis treatment, to know the changes in the life of the young adult with chronic renal failure after starting hemodialysis treatment and to identify the strategies used by the young adult for better acceptance and adaptation to hemodialysis treatment. This is a descriptive research with a qualitative approach, carried out with eight young adults with chronic renal failure of both genders who undergo hemodialysis therapy. The chosen collection technique was the snowball, where each participant indicated another by means of his or her telephone contact, the first member of the research being known to the researchers. The research was conducted through the Google Forms platform by means of an online questionnaire containing 20 mixed questions of easy resolution that were later analyzed through the Minayo's analysis technique. In this study there was a higher prevalence of female participants with age range varying from 19 to 29 years. It was observed that hemodialysis treatment has a significant impact on the daily life of young adults and that the biggest difficulties are related to daily habits, such as nutrition, decreased fluid intake. Furthermore, the time spent on the hemodialysis sessions and the discomfort during and after the treatment have repercussions on the leisure activities and on the social bond of these individuals, and in the face of these difficulties several negative feelings may arise such as sadness, fear and anguish, even knowing the importance of hemodialysis for their survival. However it was noticed that the family, the religious belief and hope in better days are configured as indispensable tools in the process of acceptance and adaptation to hemodialysis treatment. It is evident the need for the participation of a qualified nursing team in order to welcome the patient, identifying their difficulties and promoting health education actions that aim to minimize these feelings.
