Uso do tecido adiposo bucal no tratamento das fístulas bucosinusais causadas por exodontias: revisão de literatura

Abstract

A comunicação bucosinusal é uma condição patológica, existente entre o seio maxi lar e a cavidade oral, podendo evoluir para uma fístula bucosinusal, quando não di agnosticada precocemente. Esta condição pode surgir em decorrência de extrações dentárias ou a partir de procedimentos cirúrgicos para a remoção de tumores, ou ainda mediante a presença de infecção no local. O diagnóstico dessa patologia deve ser realizado nas primeiras 24 até 48 horas, por meio de exame clínico, ou ainda uti lizando exames de imagens, como a panorâmica e radiografia periapical. O trata mento da comunicação bucosinusal vai depender do diâmetro da comunicação. Em casos onde o diâmetro for menor que 2mm o encerramento da comunicação ocorre de forma espontânea, mas para as comunicações de diâmetro maior que 2mm, ex ige-se o tratamento cirúrgico. Diante deste cenário, há uma variedade de aborda gens para tratar este problema. Nesse sentido, objetivo analisar o uso do tecido adi poso bucal no tratamento das fístulas bucosinusais quanto as suas vantagens e limi tações. A presente revisão de literatura é do tipo integrativa, composta por 18 artigos científicos disponíveis em bases eletrônicas na Medical Literature Analysis and Re trievel System Online e da Biblioteca Virtual de Saúde, tendo como recorte temporal publicações de janeiro 2015 à maio de 2021. Para realização da pesquisa, foram uti lizados os seguintes descritores: Comunicação sinusal (Sinus comunication), Corpo Adiposo Oral (Oral fat pad), Fístula Oroantral (Oroantral fistula). Entre os critérios inclusivos para seleção dos textos, dentro do recorte temporal, usamos os seguintes: o formato do texto deve ser um artigo; estar escrito em um dos dois idiomas (Por tuguês e Inglês); e que os artigos estivessem disponíveis na íntegra e fossem in dexados nas bases de dados selecionadas. Foi verificado que o tecido adiposo bu cal para o tratamento das fístulas bucosinusais possui altas taxas de sucessos, dado sua excelente vantagem durante e pós-cirurgia. A pesquisa demonstra que a ação de minimizar as desvantagens deve ser evitada nestes procedimentos, pois quando se tornam realidade, elas trazem consequências graves e permanentes ao paciente. _______________________________________________________________________________________ ABSTRACT : The bucosinusal communication is a pathological condition, present between the maxillary sinus and the oral cavity, which can evolve to an bucosinusal fistula, when not diagnosed early. This condition can emerge as a result of tooth extractions or from surgical procedures to remove tumors, or even with the occurrence of infection in the site. The diagnosis of this pathology must be carried out within the first 24 to 48 hours, through clinical examination, or even using imaging tests, such as panoramic and periapical radiography. The treatment of oral-sinusal communication will depend on the diameter of the communication. In cases where the diameter is less than 2mm, the communication closure occurs spontaneously, but for communications with a diameter greater than 2mm, surgical treatment is required. Faced with this sce nario, there are a variety of approaches to address this problem. Our aim is, through out a literature review, to present on one of these approaches, namely, the use of buccal fat pad to treat the mentioned pathological condition, since this approach has become the predominant practice. The buccal fat pad is a type of graft that can be used in the treatment of oroantral fistulas, and has more advantages than disadvan tages when chose as the treatment. For this reason, it won the sympathy of the den tal community. However, it is necessary to emphasize what exactly these advantages are and, in particular, not to reduce the presence and extent of the disadvantages. This being our greatest contribution to the result of this work, i.e., by offering clarity and information for dental practice regarding both aspects of the use of oral fat pad. The type of this literature review is integrative, composed of empirical and non-empir ical scientific research articles, available in electronic databases such as the US Na tional Library of Medicine National Institutes of Health and the Biblioteca Virtual de Saúde, having as time frame publications the following: from January 2015 to May 2021. To carry out the research, the following descriptors were used: Sinus commu nication, Oral fat pad, Oroantral fistula. Among the applied criteria for selecting the texts, within the time established frame, we used the following: (a) the text format must be an article; (b) be written in one of these two languages (Portuguese and English); and (c) that the articles were available in their entirety and were indexed in the selected databases. As a result of applying these criteria, 18 articles were select ed. From these articles, it was possible to accurately draw a picture of the state-of the-art of the problem at hand. This allowed us to conclude that the oral fat pad for the treatment of bucosinusal fistulas has high success rates, given its excellent ad vantage during and after surgery. Even so, our research shows that the action of min imize the disadvantages must be avoid in these procedures, because when they be come reality, they bring serious and permanent consequences to the patient.
