Efeitos da laserterapia de baixa intensidade em pacientes com osteoartrite do joelho: uma revisão de literatura

Abstract

A articulação do joelho apresenta uma elevada frequência de lesões visto que é bem exigida na deambulação, em atividades de vida diária e na maioria dos esportes. Dentre as lesões, destaca-se a osteoartrite que pode ter como resultado a incapacidade física. Independente do grau que a osteoartrite esteja, o tratamento inicial mais indicado é o não farmacológico como, por exemplo, o fisioterapêutico que pode utilizar protocolos de laserterapia de baixa intensidade. A técnica vem sendo investigada e usada clinicamente em condições inflamatórias e infecciosas por mais de 30 anos. Com isso, o objetivo geral deste estudo foi identificar os efeitos da laserterapia de baixa intensidade em pacientes com osteoartrite do joelho. E como objetivos específicos, descrever os protocolos/parâmetros da laserterapia de baixa intensidade utilizados no tratamento da osteoartrite de joelho, relatar quais recursos terapêuticos são associados a laserterapia na reabilitação de pacientes com osteoartrite de joelho; apresentar os benefícios da laserterapia na dor e edema destes pacientes. Foi realizada uma revisão de literatura descritiva que por meio de um quadro relacionou os artigos acerca da aplicação da técnica laserterapia de baixa intensidade em pacientes com osteoartrite de joelho. Os artigos selecionados foram buscados a partir de três bases de dados: Scielo, PubMed e PEDro, utilizando os descritores “Terapia a Laser de Baixa Potência”; “Osteoartrite do Joelho”; “Low Level Laser Therapy” e “Knee Osteoarthritis”. Houve a inclusão dos trabalhos publicados entre os anos de 2011 e 2021, disponíveis de forma completa e gratuita, escritos nos idiomas português e inglês. A exclusão ocorreu àqueles que não contemplaram o tema do estudo, inconclusivos ou que apresentaram intervenções realizadas por profissionais que não sejam fisioterapeutas, teses, dissertações e revisões de literatura. Foram encontrados um total de 2.371 artigos que tratavam do tema escolhido. Após o processo metodológico estabelecido, foram selecionados 12 artigos. As características desses estudos selecionados foram expostas nos quadros 1 e 2. Foi visto que os recursos terapêuticos associados a laserterapia na reabilitação desses pacientes foram o exercício terapêutico, ultrassom terapêutico, estimulação elétrica neuromuscular e a Monochromatic Infrared Photo Energy (MIPE). Notou se que os protocolos/parâmetros da laserterapia de baixa intensidade definidos pela World Association for Photobiomodulation Therapy (WALT) obtiveram os maiores resultados positivos, sendo eles a redução da dor e melhora na funcionalidade, atentando-se ao fato de que o sintoma edema não foi analisado por nenhum dos artigos selecionados pela revisão. Com isso, essa revisão conclui que há efeitos antiinflamatórios e analgésicos no uso da laserterapia de baixa intensidade para pacientes com osteoartrite do joelho. ________________________________________________________________________________________ ABSTRACT The knee joint presents a high frequency of injuries since it is well required in ambulation, activities of daily living and in most sports. Among the lesions, osteoarthritis is highlighted, which may result in physical disability. Regardless of the degree to which osteoarthritis is, the most indicated initial treatment is non pharmacological treatment, such as physical therapy that can use low-intensity laser therapy protocols. The technique has been investigated and used clinically in inflammatory and infectious conditions for over 30 years. Thus, the general objective of this study was to identify the effects of low-level laser therapy in patients with knee osteoarthritis. And as specific objectives, to describe the protocols/parameters of low intensity laser therapy used in the treatment of knee osteoarthritis, to report which therapeutic resources are associated with laser therapy in the rehabilitation of patients with knee osteoarthritis; the benefits of laser therapy in the pain and edema of these patients. A descriptive literature review was carried out which, through a table, listed the articles about the application of the low-level laser therapy technique in patients with knee osteoarthritis. The selected articles were searched from three databases: Scielo, PubMed and PEDro, using the descriptors “Low Power Laser Therapy”; “Osteoarthritis of the Knee”; “Low Level Laser Therapy” and “Knee Osteoarthritis”. There was the inclusion of works published between the years 2011 and 2021, available completely and free of charge, written in Portuguese and English. The exclusion occurred to those who did not contemplate the study theme, who were inconclusive or who presented interventions carried out by professionals other than physical therapists, theses, dissertations and literature reviews. A total of 2,371 articles were found that dealt with the chosen topic. After the established methodological process, 12 articles were selected. The characteristics of these selected studies were shown in tables 1 and 2. This review identified the anti-inflammatory and analgesic effects of low-level laser therapy for patients with knee osteoarthritis. It was seen that the therapeutic resources associated with laser therapy in the rehabilitation of these patients were therapeutic exercise, therapeutic ultrasound, neuromuscular electrical stimulation and Monochromatic Infrared Photo Energy (MIPE). It was observed that the protocols/parameters of low intensity laser therapy defined by the World Association for Photobiomodulation Therapy (WALT) obtained the highest positive results, being pain reduction and improvement in functionality, noting the fact that the edema symptom was not analyzed by any of the articles selected by the review. Thus, this review concludes that there are anti-inflammatory and analgesic effects on the use of low-intensity laser therapy for patients with knee osteoarthritis.
