A importância do aleitamento materno exclusivo na formação da microbiota intestinal: revisão de literatura

Abstract

Assegurado pelo Estatuto da Criança e do Adolescente (ECA) a amamentação é uma forma eficaz de promoção à saúde, prevenção e redução da mortalidade infantil. O leite materno é rico em anticorpos, proteínas e outras substâncias, que contribuirão na formação da microbiota intestinal, essenciais para o crescimento saudável da criança. Ademais, a sua constituição é adequada ao metabolismo do recém-nascido e de fácil digestão. Este estudo teve como objetivo geral investigar na literatura científica as principais interações entre o aleitamento materno e a microbiota intestinal de lactentes e, como objetivos específicos, evidenciar os fatores determinantes na formação da microbiota intestinal, analisar os benefícios da amamentação exclusiva na saúde da lactante e do lactente, identificar a relação da alimentação da nutriz com a composição do leite materno. Trata-se de uma revisão de literatura integrativa, com abordagem qualitativa. A coleta de dados ocorreu no período de fevereiro a março de 2023, e, para isso, foram utilizadas as seguintes bases de dados: BVS, Scielo, Pubmed e Science Direct. Foram selecionados artigos na íntegra nos idiomas inglês e português publicados nos últimos 6 anos que atenderam os critérios de inclusão, para isso os seguintes descritores foram utilizados: microbiota intestinal e disbiose, aleitamento materno e microbiota, constituição do leite humano, aleitamento e sistema imunológico, alimentação pós-parto, alimentação e lactação. As buscas nas bases de dados resultaram na seleção de 17 estudos. Após a realização da leitura na íntegra e aplicação dos critérios de inclusão e exclusão, obteve-se um total de 8 artigos, que foram selecionados para compor essa pesquisa. Conclui-se que o aleitamento materno exclusivo contribui positivamente para o desenvolvimento e crescimento dos lactentes, além de ser essencial para a colonização do trato gastrointestinal e consequentemente no desenvolvimento da barreira imunológica, sistema nervoso e estado nutricional. ________________________________________________________________________________________ ABSTRACT Assured by the Child and Adolescent Statute (ECA), breastfeeding is an effective way of promoting health, preventing and reducing child mortality. Breast milk is rich in antibodies, proteins and other substances, which will contribute to the formation of the intestinal microbiota, essential for the healthy growth of the child. In addition, its constitution is suitable for the metabolism of the newborn and is easily digested. The general objective of this study was to investigate in the scientific literature the main interactions between breastfeeding and the intestinal microbiota of infants and, as specific objectives, to highlight the determining factors in the formation of the intestinal microbiota, to analyze the benefits of exclusive breastfeeding in the health of the lactating woman and of the infant, to identify the relationship between the nursing mother's diet and the composition of breast milk. This is an integrative literature review, with a qualitative approach. Data collection took place from February to March 2023, and, for this, the following databases were used: BVS, Scielo, Pubmed and Science Direct. Full articles in English and Portuguese published in the last 6 years that met the inclusion criteria were selected, for which the following descriptors were used: intestinal microbiota and dysbiosis, breastfeeding and microbiota, constitution of human milk, breastfeeding and immune system, postpartum feeding, feeding and lactation. Database searches resulted in the selection of 17 studies. After reading in full and applying the inclusion and exclusion criteria, a total of 8 articles were obtained, which were selected to compose this research. It is concluded that exclusive breastfeeding contributes positively to the development and growth of infants, in addition to being essential for the colonization of the gastrointestinal tract and consequently in the development of the immune barrier, nervous system and nutritional status.
